• Completed CPS Family Income Information Form 2021-2022
• Completed CPS School Enrollment Form (First Time Enrollment)
• Completed Medical Documents:
  • State of Illinois Certificate of Child Health Examination
  • Request for Emergency Health Information
• Signed Media Consent Form
• Signed School Messaging Consent Form
• Cash Fee Payment for 2021-2022 1 Student Fee ($100)

The 2022-2023 Student Health and School Form Booklet is available in English and Spanish.  Hardcopies are available in the main office.  Thank you.
